If birth of child or person is registered in urban area of the state then the office of respective Municipal Corporation/Council/Nagar Panchayat may be contacted or if the birth is registered in rural area then the office of respective Civil Surgeon may

Answers:
Application on plain paper, birth certificate if issued earlier, secondary proof of date of birth like copy of school certificate or passport or any other document on which the name of person and date of birth is written and late fee of Rs.5/-.
Q : Who can record the name of child in birth record?
Answers:
Under Rule 10 of Punjab Registration of Births & Deaths Rules, 2004, the parents or guardian can record the name of child in birth record.
Q : Whether any time limit is prescribed for registration of birth to claim benefit under this special exemption?
Answers:
No, there is no time limit or to say in other words that the birth of child could be registered in any year say 1965 or 1970 but the condition is that the birth must be registered in the birth register in which the column regarding name of child is left blank.
Q : Whether it is possible to register birth of child without name of child?
Answers:
Yes, the birth of child can be registered without name of child because sometimes the child was not given the real name immediately after the birth or in some cases the child's name is given after following some religious ceremony etc.

Answers:
Before the notification of this special exemption, if the birth of child was recorded without the name of child in Birth Record then the child's name could be recorded within 15 years from the date of registration of birth of child under Rule 10 of Punjab Registration of Births & Deaths Rules, 2004.

This special exemption is available to those children or persons whose births have been registered in the birth record of Registrar, Births & Deaths of Punjab State but without the name of child.
